Athletic Physical Forms

INITIAL EVALUATION: Prior to any student participating in Practices, Inter-School Practices, Scrimmages, and/or Contests, at any PIAA member school in any school year, the student is required to complete Packet 1 - Comprehensive Initial Pre-Participation Physical Evaluation (CIPPE). If they get their physical exam by their own physician that physician must be an Authorized Medical Examiner (AME). The list of AME’s is found at the bottom of section 6.  Physicals cannot be dated before June 1st.

Packet 1 must be turned in to the Head coach or Athletic Office for retention by the school. The CIPPE may not be authorized by an AME earlier than June 1st and shall be effective, regardless of when performed during a school year, until the next May 31st.

SUBSEQUENT SPORT(S) IN THE SAME SCHOOL YEAR: Following completion of a CIPPE, the same student seeking to participate in Practices, Inter-School Practices, Scrimmages, and/or Contests in subsequent sport(s) in the same school year, and must complete Packet 2 – Recertification physical paper, and turn it in to the Head Coach or Athletic Office.

These forms are needed for ALL Fall athletes and any first time athlete in the winter and spring seasons.  The Drug and Alcohol Information pages do not need to be printed.

 

NOTE: if you are copying your completed CIPPE printed forms, please do not make the copies double-sided. Each form goes to a specific department and it slows the process if we need to make individual copies. Each CIPPE form must be on a single sheet of paper. Thank you.

These forms are needed for all athletes who have completed the comprehensive initial physical for their first sport participation of the school year.
